This questionnaire is designed to help you observe and reflect on how a person responds to different types of sensory input. Developed by an occupational therapist and a special class teacher, it offers practical strategies that may be useful while awaiting a full occupational therapy assessment. Please note: this is not intended as a diagnostic tool or a substitute for a formal occupational therapy program. Everyone has a unique sensory profile, and noticing patterns can help you provide the right kind of support.
Primary-Aged Resource: This version is intended for primary-school-aged children. Use the post-primary version for older pupils.
A Note on Answering: Please answer these questions based on the individual's observable behaviours and preferences, within the context of their own unique abilities. The focus is on their sensory experience, not their physical or verbal skill level. Some of these questions may not be relevant to a child with physical or cognitive differences. Please choose "unsure" for these questions.

The Scores & Activity Plan:
The bar chart will show you how they scored in each area. Note: A person may show signs of more than one sensory pattern in a single area (e.g. both avoiding and seeking in touch). For any areas with a High or Moderate score, a personalised monthly activity plan will be automatically generated in the "Activity Plan" tab of your results.
Understanding the Results:
In the upcoming print menu, use the Destination dropdown menu to either choose a printer or select Save as PDF.
